• Texas Medical License: You must have a Texas Medical License if you plan to moonlight at TSRHC during your fellowship. Refer to the moonlighting physicians page for more information.
• Physician-in-training Permit: If you do not plan to moonlight at TSRHC, a Physician-in-training Permit (PIT) is acceptable and much more affordable and easy to obtain than a Texas Medical License. TSRHC will submit key information about you to the Texas Medical Board (TMB), and they will notify us of your individual TMB number. Phyllis Cuesta will e-mail you this number, which will give you access to the Texas online application. Once you’ve completed the online application, you may be required to submit additional documentation to the TMB, particularly if you have had a lawsuit filed against you or if you have any gaps in your training experience. Please submit all requested documentation to the TMB as soon as possible to avoid unnecessary delays in issuance of your permit. You will be required to pay the $144 application fee online. Please print a receipt for this expense and forward it to Phyllis Cuesta for reimbursement. When the PIT is issued by the TMB, please forward a copy to Phyllis Cuesta. This process can take up to eight weeks to complete, and it must begin after April 1 if your scheduled start date is August 1.
