Rotating Fellows and Residents From Outside Training Programs

Fellows and residents from outside residency and/or fellowship training programs who are scheduled to rotate at TSRHC must provide the following documentation to the appropriate TSRHC staff member:

  1. Fellowship in Pediatric Orthopaedics and Scoliosis

    • Copy of either a valid and current Texas Medical License OR Texas Physician-in-training Permit (PIT) issued in the name of the fellow by the Texas Medical Board.
    • DEA certificate and DPS certificate. This is required only if you have a Texas Medical License.
    • Your National Practitioner Identification Number
    • Written verification of the following insurance coverage during your TSRHC rotation:
      a. professional malpractice liability insurance with limits of coverage at $1 million per occurrence and $3 million aggregate;
      b. health insurance coverage; and
      c. workers’ compensation coverage.
    • Immunization record: Please complete and submit this form. All TB tests must be taken and dated within one year of your start date.
    • Original Consent and Release signed by each fellow or resident for our use in establishing primary source verification of medical school and residency training.
    • Driver’s license: Please submit a copy regardless of the state in which it is issued. (Passport is an acceptable alternative if driver’s license is unavailable.)
    • Social Security Card: Please submit a copy.
    • Curriculum Vitae: Please submit a copy.
    • Children’s rotation application packet (if applicable): If you are scheduled to rotate at Children’s Medical Center Dallas during your TSRHC rotation, you must also be credentialed at Children’s Medical Center Dallas and will be required to complete all forms in this packet and submit to Phyllis Cuesta. The packet of forms will be sent to you either by e-mail or U.S. mail.
