Moonlighting Physicians

The opportunity to moonlight at Texas Scottish Rite Hospital for Children is available to qualifying pediatric orthopaedic fellows. You are not authorized to moonlight if you are here on a J-1 Visa. Please contact Phyllis Cuesta in Orthopaedics if you have questions about TSRHC’s moonlighting opportunities.

Credentialing Requirements to Moonlight at TSRHC

  • Obtain a valid Texas Medical License. (Plan to start this process approximately one year prior to the beginning of your fellowship rotation.)
  • Obtain a DEA certificate (If you already have a DEA certificate, you must request a change of address to TSRHC prior to starting your rotation. If you do not have one, you may apply online after obtaining a Texas Medical License at www.deadiversion.usdoj.gov/online_forms.htm.)
  • Obtain a Texas Department of Public Safety (DPS) certificate. You must have your Texas medical license and DEA certificate before you can apply for this certificate. Application forms can be obtained at www.txdps.state.tx.us.
  • TSRHC credentials are required to gain privileges as TSRHC on-call consulting staff. The credentialing packet can be obtained from the TSRHC credentialing coordinator, Rebecca Polk. Please return the completed packet to:

    Note: This process can take several months to complete.
  • TSRHC employment forms (if applicable). Moonlighting physicians must become employees of TSRHC in order to be covered by the hospital’s professional liability insurance policy. Please contact Phyllis Cuesta at (214) 559-7604 for employment forms.
