International Graduates
In addition to the documents in the checklist for fellows and residents, please provide a copy of the items listed below that are applicable to you.
To obtain a J-1 Visa through the ECFMG
Step 1: Copies of the checklists and application forms for initial J-1 Visa sponsorship in accredited clinical training programs and non-standard clinical training programs can be accessed on ECFMG’s Web site. Please return all of the required documentation to Phyllis Cuesta. TSRHC will provide you with a contract or letter of offer. View fellowship program descriptions.

Step 2: Your documentation will be submitted by Phyllis Cuesta to the department of Orthopaedic Surgery at UT Southwestern Medical Center at Dallas and forwarded to the International Affairs Office at UT Southwestern Medical Center at Dallas. The International Affairs Office will function as your Training Program Liaison (TPL) and will submit the documentation to the ECFMG. Upon establishing the applicant’s eligibility, ECFMG will issue form DS-2019, J-1 Certificate of Eligibility for Exchange Visitor status, as well as forms for dependents (if applicable). Issuance of form DS-2019 indicates that the physician is now eligible to apply for the J-1 Visa.
Upon entry into the U.S. and arrival to Dallas, the fellow will be required to personally meet with the TPL at UT Southwestern and provide TSRHC and UT Southwestern with copies of DS-2019, I-94, a copy of his or her passport and a government-issued photo ID.

- THECB Coursework Verification Form: This applies only to those graduating from a foreign medical school. Please have this form completed by your medical school and returned directly to:
