Welcome Future Fellows and Residents!

Congratulations to the orthopaedic residents who have been selected for a fellowship position at Texas Scottish Rite Hospital for Children! We also welcome the orthopaedic residents who are scheduled to rotate at TSRHC. You are now a part of the TSRHC family, and we look forward to having you at the hospital.

The following checklist includes a detailed description of the steps you will need to take before you begin your fellowship or rotation at TSRHC. All applicable forms listed on this checklist MUST be completed (please type or print clearly) and returned to one of the following individuals:

Checklist for Fellows and Residents

  • TSRHC Employment Application and Employment Forms (if applicable): All future Edwards Fellows, McStay International Fellows and Hand Fellows will be employed by TSRHC and must complete and submit the application. Please note that all prospective TSRHC staff must undergo a drug screen and criminal background check prior to employment.
    • Personal Information Update Form
    • Form W-4 Employee's Withholding Allowance Certificate
    • U.S. Department of Justice Employment Eligibility Form (I-9): Please complete Sections 1 & 2 and attach copies of the appropriate documents to verify eligibility. A list of acceptable items is listed on the reverse side of the I-9 Form.
    • Worker's Compensation Health Care Network Employee Acknowledgement Form. Please complete, sign and return this form. The information handout is for you to keep.
    • Worker's Compensation Information Form. Please sign and return this form.
    • Electronic Communications Policy - Staff Acknowledgment and Agreement. Please sign and return this form.
    • Intellectual Property Policy and Procedure. Please sign and return this form. The copy of the hospital's Intellectual Property Policy and Procedure is for you to keep.
    • Hospital Attendance Guidelines. Please sign and return this form.
    • Information about the hospital's 403(b) provider. Please keep this form.
  • DEA Certificate: This is required only if you have a Texas Medical License, but if you already have a DEA certificate, the address must be changed to Texas Scottish Rite Hospital for Children's address prior to beginning your rotation.
  • Medical School Diploma: Please submit two notarized copies indicating that it is a true and correct copy of the original document.
  • Medical School Transcripts: Request your medical school registrar's office to send a FINAL transcript directly to Texas Scottish Rite Hospital for Children, Attn: Phyllis Cuesta, administrative director, Orthopaedics, 2222 Welborn Street, Dallas, Texas 75219.
  • USMLE Scores: Please submit copies of test results of Steps I and II (required) and Step III (if available).
  • Residency Certificate: Please submit a copy.
  • Your National Practitioner Identification Number
  • Original Consent and Release. Please sign and return this form for us to use to conduct primary source verification of your medical school and residency training. This form will be sent to you either by e-mail or U.S. mail.
  • Driver's License: Please submit a copy regardless of the state in which it is issued. (Passport is an acceptable alternative if driver's license is unavailable.)
  • Social Security Card: Please submit a copy.
  • Immunization Record: Please complete and submit this form. (All TB tests must be taken and dated within one year of your start date. This can be done at TSRHC by our staff health nurse when you get here, if necessary.)
  • Curriculum Vitae: Please submit a copy.
  • Children's Rotation Application Packet (if applicable): If you are an Edwards or McStay Fellow, you will be required to do a one-month rotation at Children's Medical Center Dallas in their Level 1 Pediatric Trauma Center. As a result, you must be credentialed at Children's Medical Center Dallas. This packet of forms will be sent to you either by e-mail or U.S. mail. Please complete all forms in this packet and submit to Phyllis Cuesta.
  • Copy of either a valid or current Texas Medical License or Texas Physician-in-Training Permit (PIT) issued in the name of the fellow by the Texas Medical Board.
  • DPS Certificate: This is required only if you have a Texas medical license.
