General Orthopedics / Scoliosis

If you are looking to apply for treatment in General Orthopedics / Scoliosis at TSRHC’s main campus in Dallas, please obtain a completed and signed physician referral with our Patient Referral Information Form:


Dyslexia & Learning Disorders

If you are looking to apply for our Luke Waites Center for Dyslexia and Learning Disorders located in TSRHC’s main campus in Dallas, please complete our Application for Evaluation in the Center for Dyslexia:


Sports Medicine Center

If you are looking to request an appointment for a sports-related injury or problem at our Sports Medicine Center in Plano, contact us through our online form or call 469-515-7100:

 

Fracture Clinic 

If you are looking to request an appointment for a fracture, or broken bone, visit our Fracture Clinic page or call 469-515-7200.
